Landing at 28 1900Z. Which line applies, and do we need an alternate?
All 3 lines need to be considered due to the ETA ±1 hour rule. TEMPO only modifies certain elements of the underlying TAF and BECMG becomes effective by the end time, it's a timing trap.
Alternate required. The forecast includes a ceiling below 2,000 feet AGL and visibility below 3 statute miles within the required window. (202V3, 4.16.2.1)

4.16.7. (Added-AMC) File two alternates when filing to a destination where (based on the lowest compatible approach):
4.16.7.1. (Added-AMC) The forecast visibility (intermittent or prevailing) is less than published for the available precision approach. (T-2)
4.16.7.2. (Added-AMC) The forecast ceiling OR visibility (intermittent or prevailing) is less than published for all other approaches. (T-2)
Departure Alternates
5.2. (AMC) Weather. MAF aircraft may depart when takeoff weather is below approach minimums (ceiling or visibility), but a departure alternate is required.
5.2.2. (Added-AMC) A departure alternate is required if weather at the departure airport is below the lowest landing minimums for a suitable approach (ceiling or visibility, or crosswinds). (T-2) If planning an ILS approach, Cat I minimums will be used. (T-2)
